Box 750 Plainville, CT 06062 plainvillecolts@hotmail.com	Official Website Plainville Colts Football & Cheerleading, LLC The Plainville Colts is a non-profit youth football and spirit (cheerleading) organization serving young athletes, ages 5-15 years old, in the Plainville/Farmington and surrounding areas. We are a member of the Western Connecticut Pop Warner League, a subsidiary of the Pop Warner National Football and Cheerleading Organization. The league focuses on building strength in character of both mind and body through youth athletics. The Colts will be having OPEN REGISTRATION July 15th at the Plainville Rec. Dept. On Whiting St. from 6:00-8:00 pm Forms for the 2008 Season can be downloaded using the links located on the left….. Additional IMPORTANT information for Parents, Players & Cheerleader --- · Player/Cheerleader will need to be present for uniform/gear distribution on August 2. · Further cheer accessories will need to be purchased by the participant (i.e.: lolly, crop top, sneakers, hair piece, hair bow, socks – purchase info will be submitted the first week of practice) · All paperwork/payments must be submitted - as you will be ineligible to take the field without submittal. Payment – Registration & Fundraising ** (see below for further information) Birth Certificate (league officials need to view original, please bring a copy and the original) a. Player contract (filled out at registration) b. Medical form – signed and dated (2008) by physician c. Report card – 07/08 final report card issued in June **Please be prepared to purchase a book of raffle tickets prior to or on the first practice. The fee is $50.00 for the tickets, you will receive 20 tickets to be sold at $5.00 each; when you have sold your entire book you have $100.00 (this reduces your registration fee to $100.00). Raffle tickets will be collected at a date to be determined. If you elect not to sell the raffle tickets, please just put your name on all stubs and turn them, as you have already purchased them. Please be sure that ticket stubs are completely filled and are legible. Practice will begin August 4 – Gear will be distributed at the Shed at Norton Park on Saturday, August 2 – NO GEAR WILL BE ISSUED UNTIL FEES ARE PAID.
